Reminder that ALL students participating in a Monrovia MS or Monrovia HS sport MUST have a physical on file that is dated 4/1/23 or later for the 2023-2024 school year. This includes summer workouts! Please SEE the dates and opportunities listed below and attached to sign up today for your physical! We also have switched to a website called FinalForms (previous was RankOne) to fill out the additional online forms that are required to go along with the physical. Those instructions are attached. * If you are attending an appointment at a Hendricks Regional Health location, you will need to print out the Pre-Participation Physical Form that is attached and take it to your appointment * If you are attending at appointment at the Stop In Clinic in Monrovia, you will NOT need this form prior as they will have copies for you on hand What you will see attached 1) Hendricks Regional Health dates/times to sign-up for physicals at their locations 2) Stop in Clinic Monrovia dates/times for physicals (sign-up by emailing athletic dept) 3) Pre-Participation Physical Form that is REQUIRED to bring with you to a Hendricks Regional Health appointment (Stop In Clinic appointments will have these on hand) 4) FinalForms instructions to complete your online paperwork needed
