Reminder that ALL students participating in a Monrovia MS or Monrovia HS sport MUST have a physical on file that is dated 4/1/24 or later for the 2024-2025 school year. This includes summer workouts! Please SEE the dates and opportunities listed below and attached to sign up today for your physical! Hendricks Regional Health is $25 and a portion of the proceeds go back to the school. Stop in Clinic Monrovia (317-996-4223) is also an option if you'd like to stay closer and pay a little more ($40). We will be using Final Forms again. Instructions to fill out the additional online forms that are required to go along with the physical are attached. If you created an account last year, this should be a much easier process. REMINDER that you don't "turn in" the physical to us, but you upload it on the Final Forms site.
